To get an access card, fill out the form below and mail or fax to Alan Roberds of ASI at 1040 William Hilton Parkway Suite 200, Hilton Head, SC 29928.  Fax number:  843-785-4951.  The form will be processed when received and the information for reactivation or activation of a new card will be sent to Joel Horton from Chapel Park.  If a card is lost, there is a $15 charge to replace your card that must be paid prior to activation.  Amenity cards will only be issued and activated to homeowners who are current in dues.
